A life without glasses or contact lenses – that is what many people, for whom visual aids sometimes mean considerable restrictions in their personal well-being or in their professional life. Existing myopia (nearsightedness) or hyperopia (farsightedness) with or without astigmatism can be corrected through various methods.

Refractive surgery means eye operations that change the overall refractive power of the eye and thus replace or significantly reduce the diopters of visual aids such as glasses or contact lenses. There are operations that change the refractive power of the cornea (e.g. Femto-LASIK, Trans PRK, lenticle extraction) and so-called lens procedures, which change the refractive power of the eye by additionally inserting, exchanging or removing the lens (phakic intraocular lenses, refractive lens exchange).

For different methods, your eyes have to meet different conditions (stability of ametropia, corneal thickness or anatomy of the eye, accompanying diseases,…) – not every eye is suitable for eye laser surgery.

Beispiel Femto LASIK Augenlaser

With femto-LASIK, a 0.15 mm thin lamella of the cornea is first cut with a femtosecond laser and unfolded. Corneal tissue is then removed using an excimer laser to compensate for the ametropia. The corneal flap is then folded back in place. It adheres tightly itself and therefore does not have to be sutured.

TransPRK (Transepithelial Photorefractive Keratectomy) is a laser procedure that is applied directly to the surface of the cornea. In this treatment, the superficial layer of the cornea (the epithelium) is removed with the laser. This means that there is no need for a corneal flap, as is required with femto-LASIK. The laser treatment to correct ametropia is performed on the corneal surface. The epithelium reforms in a few days; thus the superficial wound closes by itself.

Although the visual results are the same compared to the LASIK method, a slower and more uncomfortable healing process must be expected with surface procedures such as TransPRK. In order to facilitate the healing process and to protect the eye optimally, the patient is given a soft contact lens for the first few days after surgery, as a kind of transparent bandage. This helps the cornea to regenerate quickly. In addition, the discomfort is usually quite slight, temporary and well controllable through medication.

The latest development in refractive surgery is lenticular extraction using a femtosecond laser for the correction of myopia and astigmatism (e.g. SmartSight). The laser cuts a pre-calculated lenticule within the tissue of the cornea and dissects a small peripheral access. After the laser treatment, the lenticle is removed through the small tunnel. With this procedure, no flap has to be cut.

Using a refractive corneal laser, we have been able to correct presbyopia using an excimer laser (Presbyopia LASIK, e.g. PresbyMAX) for several years.

The principle of this method is to model the corneal surface: the center for near vision and the periphery for distance vision. This results in good visual acuity at all distances – whether near or far. There are various treatment options for the PresbyMAX method, which I would be happy to inform you about.

Phakic lenses, i.e. implants that are used in addition to the body’s own lens, are used to correct higher myopia and higher hyperopia, especially when corneal surgery such as LASIK is not possible.

The eye is first opened at the margin of the cornea. An additional lens is then placed in the eye. This additional lens is attached to the iris. Other models sit in front of the lens behind the iris. After the lens is inserted, the incision either seals itself or is closed with a suture.

Beim Linsentausch wird die körpereigene klare Linse gegen eine Kunstlinse ausgetauscht.
Zunächst wird das Auge am Rand der Hornhaut eröffnet. In einem weiteren Schritt wird, wie bei der Grauen Star Operation, die Augenlinse entfernt und durch eine Kunstlinse ersetzt. Nach dem Einsetzen der Kunstlinse dichtet sich der Schnitt von selbst ab.
Der Linsentausch wird zur Korrektur von Myopie, Hyperopie, Astigmatismus und Alterssichtigkeit angewandt.
